We are excited that you are interested in enrolling your senior adult into our Adult Day Program. We are certain it will benefit them as they face the challenges of aging and you as a caregiver. Here are the next steps to enroll your senior adult:

1. Give us a call at 540-403-0557 to Learn More
   We will take time to learn more about you and your senior adult and ensure that they are a good fit for our program. Have a question about fees? We will share these with you as well as any sliding scale options you may qualify for when we initially connect with you.
2. Schedule a Tour
   We will give you and your senior adult a chance to see our program in action. We will also have our Nurse Coordinator complete an initial assessment of your senior adult to ensure we can provide the care they need to thrive at the Center. We will discuss our current openings and determine a schedule for your participant to attend.
3. Schedule an Appointment with a Physician
   Each potential participant must have their doctor complete our physical examination and TB screening (listed above) in order to begin attending. Please ensure that every section of the form is complete.
4. Return the Completed Paperwork
   In addition to the physical examination and TB screening forms, we will provide you with several other forms to complete. To enroll your participant in the program, please return all enrollment paperwork to us no later than 30 days after the physical exam is completed. If the 30 day period expires, you will have to have another physical examination completed.
5. Choose a Start Date and Begin Thriving!
   Together we will determine the best start date for you and your participant. Worried about your senior adult having trouble settling in? We understand that it can take time for our participants to adjust to being away from their caregivers and their home. Our staff is very committed to supporting each participant to help settle them into our program. We also have a two week trial period that allows you to end services at any time during the first two weeks with no additional fees if you feel that we are not the right fit for you or your participant.
